In order to board my dog at Summit Kennels, I agree to the following:Please Check All BoxesWaivers and Information: *All dogs must have a complete, signed, up-to-date Boarding Intake, Policies and Procedures, Boarding Agreement, Medical Release, and current vaccination record on file.Boarding Drop-Off / Pick-Up: *Boarding drop-off and pick-up is available anytime by appointment. Dogs not picked up by 1:00 pm will be charged a full nightly rate for that day.Additional Persons Authorized to Pick up: *Additional authorized persons who are listed on the intake form as allowed to pick up the dog must provide valid ID before taking the dog.Payments: *Boarding fees may be paid with credit card, debit card, Venmo, Zelle, check, or cash. All fees must be paid in full prior to service being rendered. A refund will be issued in the case of early departure.Reservations & Cancellations: *Reservations are required. A 50% deposit is required to reserve your run. Cancellations can be made at any time and deposits are refundable. Credit/debit card payments are refunded minus a 5% processing fee.Evaluation and Group Play: *Evaluations are not required for boarding dogs. Group play sessions are NOT OFFERED for dogs. Dogs from the same household may share a run and play together as long as they get along with each other.Leash: *All dogs must be on leash when arriving or leaving the property. Dogs must be on leash prior to hopping out of the car.Age/Vaccine Requirements: *Puppies must be at least 16 weeks old to board at Summit Kennels and must have all their puppy inoculations for Rabies, Distemper, Parvovirus, Parainfluenza, Adenovirus (or Hepatitis), and Bordetella.Health: *Client certifies that the representation of the dog’s health is accurate on the intake form. Client assumes responsibility to keep Summit Kennels up to date on any changes in the dog’s health and physical condition. Client also certifies that the dog is free of fleas and ticks, and from any condition(s) that could potentially affect the health of other dogs. Dogs who have been ill with any communicable disease in the last 30 days require a veterinarian’s release to be admitted to the kennel.Medical Emergencies: *If a dog becomes ill or is deemed to need immediate medical attention Summit Kennels will administer aid and/or to use any available veterinarian. Summit Kennels’ preferred veterinarian is The Pet Doctor in Lakewood. The dog’s usual vet will be contacted if medical attention is needed. Client will be contacted if medical attention is needed. Client is responsible for all expenses incurred for medical treatment.Medication: *Oral or topical medications can be administered as instructed. A complete a Medication Intake Form is required for all medications. Please bring all medications in their original container with your dog’s name on it. On the Medication Intake Form, please include the name and strength of the medication, and directions for administering the medication, including dosage, number of times per day, how and when it is to be given, condition being treated, and areas to be treated.Aggressive Behavior: *Summit Kennels accepts any breed of dog. Summit Kennels accepts dogs that do not get along with other dogs or may be aggressive toward other dogs. Summit Kennels WILL NOT accept dogs who have shown aggression to people outside the context of sport (e.g. Schutzhund, PSA) or military and police work. Please be clear on the intake form if you dog has ANY issues with other dogs.Separation Anxiety: *Dogs who experience prolonged, severe distress when left alone do not do well in the kennel environment and will not be accepted for boarding. This can look like excessive barking, howling, whining, repeated obsessive attempts to escape, destructive behavior, self-mutilation, etc. If your dog exhibits these symptoms during their stay, you will be notified and either the dog will need to be seen by a veterinarian and prescribed medication or the dog will need to be picked up.Pregnant/In Heat Females: *Summit Kennels does not accept females who are pregnant or in heat. Intact females must be at least 4 weeks past the start of their last heat cycle and not showing signs of estrus such as swelling or bleeding.Flea Control: *Dogs must be on some type of flea control.Toys and Bedding: *Summit Kennels allows bedding as long as it can be laundered and has your last name permanently marked on it somewhere. Toys are also allowed as long as they have no squeakers. No rawhides or any toy that may be a choking hazard. Please ensure all bedding is clean and flea-free.Food When Boarding: *To prevent stomach upset it is suggested that you bring the food your dog is used to eating. Please provide complete feeding instructions on the intake form. The kennel has a kitchen with a refrigerator and multiple freezers and can accommodate a wide variety of diets and preparations.Personal Property: *Summit Kennels will not be held responsible or held liable for any lost, stolen, or damaged personal property belonging to either me or my dog. If my dog causes any damage to the Summit Kennels facility or equipment, I understand that I may be held responsible for the cost of repair or replacement.Policy Changes: *Summit Kennels reserves the right to change policies procedures, and services, and adjust rates without notice.Abandonment: *I understand that if I do not pick up my dog Summit Kennels will proceed according to the guidelines provided by Washington State Law. I also acknowledge that I will be fully responsible for all attorney’s fees and associated costs if I abandon my dog.Photos, Videos, Social Media *Summit Kennels has the right to take photographs and/or videos, and to use the images or videos of boarding dogs including the dog’s name in printed matter, internet sites, and social media (website, Facebook, Instagram, etc.), or other promotional or advertising capacities. Photographs and videos are the property of Summit Kennels.Right to Refuse Service: *Summit Kennels reserves the right to refuse or suspend any and all services to anyone for any reason in accordance with law.
